应用超声评分法对胎儿肾积水预后的研究

目的 探讨超声评分法评估胎儿肾积水预后的价值.方法 晚孕期149例肾积水胎儿,测定其肾盂前后径、肾实质厚度、肾盂肾盏形态并分别赋予0～3分,累计其分值并与出生后的超声检查及临床诊断对照.结果 超声评分为≤3、4、5、6、7、≥8分,病理性肾积水占的比例分别为0、11.11%、28.57%、50.00%、80.00%、100%.6分为诊断胎儿病理性肾积水的最佳截断值,其灵敏度、特异度分别为88.46%、94.49%.结论 超声评分法评估胎儿肾积水的预后,具有较高的临床实用价值.

Ultrasound Scoring in Predicting the Prognosis of Fetal Hydronephrosis

Objective To research the value of ultrasound scoring in evaluating the prognosis of fetal hydronephrosis.Methods Ultrasound was performed in 149 fetuses with hydronephrosis.Renal APD,parenchyma thickness and pelvicaliceal morphology were measured and were graded from 0 to 3 score.These scores were compared with post-natal ultrasound and clinical diagnosis.Results Scores were ≤3,4,5,6,7,≥8 respectively,pathological hydronephrosis rates were 0,11.11%,28.57%,50.00%,80.00%,100% respectively.Score 6 was the best cutoff to diagnose fetal pathological hydronephrosis.The sensitivity and specificity were 88.46%、94.49% respectively.Conclusions Ultrasound scoring was a feasible and accurate method to evaluate the prognosis of fetal hydronephrosis.
