慢性肾炎患者网织红细胞参数动态变化及意义

目的探讨网织红细胞(网红)参数在慢性肾炎患者中变化及临床意义。方法用COULTER-GENS.SYSTEM2五分类血细胞仪对50例慢性肾炎治疗前后及200例健康体检者的网红及红系参数进行测定,并将两者结果作比较并分析。结果慢性肾炎患者红细胞计数(RBC)和血红蛋白(Hb)治疗前比正常对照组极显著性下降(P<0.01),未成熟网织红细胞比例(IRF)和平均网织红细胞体积(MRV)有明显增高(P<0.05);治疗后IRF比对照组及治疗前极显著性升高(P<0.01),RET%、MRV、RBC、Hb比治疗前均有较明显的增加(P<0.05);将慢性肾炎治疗前后各组共100例标本的IRF与Hb、RBC进行相关分析,结果IRF与Hb、RBC均存有一定的正相关性(分别为r=0.536和r=0.231,P<0.01)。结论网红参数为反映慢性肾炎骨髓造血功能较好的指标;其中IRF为更灵敏指标,与红系参数变化具有较好相关性;网红参数测定对于慢性肾炎指导临床药物使用及预防感染、贫血、出血等具有重要意义。

The clinical significance of reticulocyte parameter changes in patients with chronic nephritis

Objective To probe into the clinical significance of reticulocyte parameter changes in patients with chronic nephritis. Methods The parameters of the reticulocytes and erythrocytes were detected from fifty patients with chronic nephritis before and after therapy and heathy person controls by Coulter-Gens system II analyzer, then all the data were analyzed. Results Compared with the control group, the chronic nephritis group with pre-therapy showed significantly decreasing in RBC and Hb respectively(P<0.01), and the immature reticulocyte fraction(IRF)and the mean reticulocyte volume(MRV) were evidently increased respectively(P<0.05); After therapy IRF increased significantly(P<0.01) and the percent of reticulocyte, MRV,RBC,Hb were showed significantly increased respectively(P<0.05). Conclusions The reticulocyte parameters, especially the IRF and the correlation of IRF and RBC's, are well indexes of showing the hematopoiesis of bone marrow in patients with chronic nephritis, It is suggested that reticulocyte parameters are important to clinical therapy and to prevent from infection, anemia and bleeding with regarding to chronic nephritis.
