摘    要:目的评价1.5TMRI引导下^125I放射性粒子组织间植入治疗恶性肿瘤的可行性。方法对44例恶性肿瘤患者(男41例,女3例)共99个病灶在1.5TMRI引导下行^125I粒子永久性组织间植入。术前采用TPS系统设计治疗计划。采用fsFRFSET2WI(30s)、T1FSPGR(16s)、2DFIESTA(1s)、3DDynT1WI(15s)引导18GMR兼容性穿刺针插入病灶,并根据TPS计划植入^125I粒子。采用WHO实体瘤疗效评价标准评价疗效。结果穿刺针与^125I粒子在MR图像上均能清楚显示。49个病灶(49.49%)完全缓解;29个病灶(29.29%)明显缓解;14个病灶(14.14%)无变化;7个病灶进展(7.07%)。未见大出血和胆瘘等严重并发症发生。结论 MRI引导下^125I粒子组织间植入是一种安全有效的治疗恶性肿瘤的手段。

Abstract:Objective To evaluate the feasibility of 1.5T MRI-guided 125I radioactive seeds percutaneous interstitial implantation in patients with malignant tumors. Methods Forty-four patients (41 male and 3 female) with a total of 99 malignant lesions were treated with 125I seed permanent interstitial implantation under the guidance of a 1.5T MR system. Before implantation, the treatment plan was designed with treatment plan system (TPS) according to CT or MRI. The sequences of fs FRFSE T2WI (30 s), T1 FSPGR (16 s), 2D FIESTA (1 s), 3D Dyn T1WI (15 s) were used to guide an 18G MR compatible needle inserted into lesions and introduce the 125I seeds according to the TPS. The WHO solid tumor response was used to evaluate the curative effect. Results The needle and 125I seeds could be clearly detected on MRI. Complete remission was seen in 49 lesions (49.49%), partial remission in 29 lesions (29.29%), no changes in 14 lesions (14.14%), while progression in 7 lesions (7.07%). No severe complication such as massive hemorrhage and bile fistulae occurred. Conclusion MRI-guided 125I seeds interstitial implantation is a safe and effective curative method in the treatment of malignant tumors.
