淋巴闪烁显像法在口腔癌前哨淋巴结活检术中的应用

目的　利用淋巴闪烁显像(LS)法进行口腔癌前哨淋巴结活检术(SLNB)术前体外定位,探讨其临床应用价 值。方法　选择10例口腔癌患者,术前1 d进行LS检查,在原发灶周缘黏膜下注射核素显影剂99mTc-Dextron,进行 LS显像。术中采用染料示踪法行前哨淋巴结(SLN)定位,并行常规颈淋巴清扫术。术后对所有淋巴结进行病理学 检查。结果　10例患者中有8例于术前探查到SLN,共计10处;此8例患者术中均探测到蓝染SLN,其中8处的术 中探查结果与术前定位完全吻合。术后病理学检查此8例患者中4例淋巴结转移阳性,术前均探测到LS浓聚点。 结论　LS法术前体外定位SLN有助于SLNB在口腔癌中的应用,可作为SLNB术前的常规定位方法。

Sentinel Lymphoscintigraphy in Oral Carcinoma: A Clinical Report of 10 Cases

Objective To determine the clinical value of preoperative in vivo location of sentinel lymphnode (SLN) by lymphoscintigraphy (LS) in oral carcinoma. Methods 10 patients with oral carcinoma were included in this study (7 male to 3 female). {99m} Tc-dextron was injected peritumorally 24h before surgery. Lymphosc intigraphy images were obtained in anterior and lateral views. SLNs were detected and removed with the aidofdye during surgery. A standard radical neck dissec tion was then performed, and all lymph nodes were analyzed by HE staining. Results SLNs were successfully detected in 10 regions of 8 patients via lymphoscintigrap hy. Consistent with the result of LS, 15 blue-stained SLNs were identified by means of dye method in 8 of 10 regions which the LSimages indicated. Conclusion This study suggests that preoperative in vivo location of SLN vialy mphoscintigraphy should be helpful for the application of SLNB in oral carcinoma.
