郁证发微(二十四)——郁证泛酸论

郁证性泛酸是指由七情内伤引起或参与引起的泛酸,可分为病郁同存与单纯郁证两种。病郁同存是指肝胃同病、肝胃不和,主要包括肝气犯胃与肝胃郁热;单纯郁证是指七情伤及肝心(脾)而表现出的相应证候。单纯郁证肝病泛酸或仅伴有少数肝经表现,或可伴有涉及多脏腑纷繁多样的躯体症状,后者多属隐性郁证或广义郁证。六郁也是郁证性泛酸的常见病机。郁证性泛酸有其临床特征,须从郁论治或辅助从郁论治。郁证性泛酸可见于现代医学精神心理因素引起的胃食管反流病、功能性消化不良以及抑郁症、焦虑症、躯体形式障碍等疾病。

Acid regurgitation due to depression syndrome

Acid regurgitation due to depression syndrome refers to acid reflux caused by the seven emotions directly and indirectly,which presents two manifestations as single depression syndrome or the coexistence of diseases and depression syndrome.The coexistence of diseases and depression syndrome refers to liver-stomach diseases and incoordination between liver and stomach,mainly including the syndromes of liver qi invading stomach and stagnated heat in liver and stomach. The single depression syndrome refers to the corresponding syndromes caused by liver and heart (spleen) impairment due to seven emotions.Acid regurgitation of single depression syndrome induced by liver diseases may present with a few manifestations in liver meridian,or various complex physical symptoms involving multiple organs.The latter mainly belongs to recessive depression syndrome or generalized depression syndrome.The six stagnation diseases are also considered as the common pathogenesis of acid regurgitation due to depression syndrome. Acid regurgitation due to depression syndrome can be identified by its clinical features.The treatment should base on relieving depression or adjunctive therapy for depression. Acid regurgitation due to depression syndrome can be seen in the diseases such as gastroesophageal reflux and functional dyspepsia,depression,anxiety and somatoform disorders,which caused by mental and psychological factors in modern medicine.
