UCH-L1在癫痫患者脑脊液和血液中的表达及意义

摘    要:目的研究泛素羧基末端水解酶L1(UCH-L1)在癫痫发作后早期患者血液和脑脊液中的表达情况,并探讨其临床意义。方法在癫痫发作后即刻和发作后12 h,采集52例癫痫患者的静脉血及脑脊液作为实验组;取19例无癫痫病史及神经系统疾病患者的静脉血及脑脊液标本作对照,采用酶联免疫吸附法检测标本中UCH-L1水平。结果在癫痫发作后即刻和发作后12h,实验组脑脊液中UCH-L1浓度均明显高于对照组(P〈0.05),而血液中UCH-L1的浓度在发作后12 h才明显高于对照组(P〈0.05)。血液与脑脊液中UCH-L1浓度与癫痫患者的年龄呈显著正相关(相关系数分别为0.55和0.63,P〈0.01)。反复发作(发作次数≥4次)的癫痫患者血液中UCH-L1的浓度要明显高于发作1次、2次的患者(P〈0.05)。结论血液和脑脊液中UCH-L1可以反映癫痫造成的大脑损害,可以为癫痫发作后早期实施脑保护措施提供参考依据。

Changes of UCH-L1 in peripheral blood and cerebrospinal fluid of patients with epilepsy

Abstract:Objective To investigate the changes of ubiquitin carboxy-terminal hydrolase L1 (UCH-L1) in the peripheral blood and the cerebrospinal fluid (CSF) of patients with epilepsy and their clinical significance. Methods Fifty-two patients with epilepsy were prospectively recruited as group A and another 19 healthy subjects were collected as control group. The levels of UCH-L1 in CSF and plasma of all subjects were detected by ELISA assays. Results The levels of UCH-L1 in CSF were significantly higher in group A both immediately and 12 hours after seizure than that in group B (P〈O.05). The level of UCH-L1 in peripheral blood in group A immediately after the seizure was similar to that in group B (P〉O.05), but it was significantly higher 12 hours after seizure in group A than that in group B (P〈O.05). The levels of UCH-L1 in CSF and in peripheral blood were significantly positively correlated with the age of patients with epilepsy (r=0.55 and r=0.63, respectively; P〈0.05). Conclusion Our results suggest that mornitoring of the levels of UCH-L1 in CSF and Deriuheral blood might held to assess the brain damage followinz seizure in patients with epilepsy.
