| 摘 要： | 目的探讨MRI膝关节周围骨质异常信号的鉴别诊断及治疗意见。方法回顾性分析80例膝关节MRI骨质信号异常患者的影像学和临床资料,并做对比分析。将膝关节骨质信号异常分为应力性骨损伤和缺血性骨坏死。结果(1)应力性骨损伤常局限于软骨下骨;缺血性骨坏死常累及干骺端及骨干。(2)应力性骨损伤常伴有关节退行性改变,关节间隙变窄,胫骨平台增生硬化,半月板撕裂和韧带损伤,关节软骨损伤,关节过度使用史;缺血性骨坏死常有激素使用史、酗酒和胰腺炎、血液病等疾病。(3)应力性骨损伤X线可见"新月征"、"关节鼠",骨关节炎软骨下假囊肿(OAPC);缺血性骨坏死的典型MRI表现为"地图样"改变。结论不同类型的膝关节MRI骨质信号异常,其影像学表现和病理基础不同,可以为临床治疗提供依据。
