动脉法数字减影血管造影的临床应用(附210例分析)

数字减影血管造影(DSA)临床应用210例共作581根次选择性血管造影:颈—椎动脉237次,腹主动脉及其分支175次,胸主动脉—头臂动脉86次,以及心脏、支气管动脉、四肢动脉等。除1例造影失败外,其余均成功,未发生严重并发症,图象优、良率分别为92％和6％。着重讨论了动脉法DSA的临床应用及提高图象质量的措施。

Clinical Application of lntraarterial Digital Subtraction Angiography: with Analysis of 210 Cases

The initial experience with intraaterial digital subtraction angiography (IADSA) in 581 examinations of 210 cases(l46 male and 64 female) are presented, including 237 examinations of carotid-vertebral arteries, 175 abdomi- nal aorta and its main branches and 86 thoracic aorta and its main branches. All the procedures were successful except for one failed. No serious complications were encounted except for some nausea and vomiting and some hematoma at the rout of punctures. Good and excellent rates of imaging quality of IADSA are 6% and 92% respectively. Comparing with intravenous DSA (IVDSA),the advantages of IADSA are: l.Good imaging quality, direct showing the arterial morphological distribution, trench and the venous condition and clear anatomical relationship. 2. Reduced contrast medium with diluted concentration. 3. Real-time visualization and dynamic observation. 4. Because of the high density resolving power, advantageous to the observation of parenchymatous organs, especially the tumor stain, portal vein ctc. 5. Very useful for interventional radiology. The measures how to improve the quality of imaging are discussed, such as, owing to the smaller visual field of IADSA, the interesting areas should be moved one by one to avoid any missing of lesions.
