丙泊酚复合布托啡诺芬太尼用于人工流产术的麻醉

目的：观察丙泊酚复合布托啡诺芬太尼用于人工流产术的临床镇痛疗效和安全性。方法：60例行无痛人工流产术患者随机分为三组,每组20例。A组,丙泊酚诱导前1min缓慢静注布托啡诺0.04mg/kg,B组,丙泊酚诱导前缓慢静注芬太尼0.002mg/kg,C组丙泊酚2mg/kg,30s内匀速输注,术中体动追加丙泊酚1mg/kg至体动消失。记录用药前、用药后2min,术毕MAP、HR、SPO2及丙泊酚用药总量,手术结束患者清醒后5、10、15 min宫缩痛VAS评分。结果：三组HR无显著差异,观察组（布托啡诺及芬太尼）用药后2 min MAP比对照组显著降低（P〈0.05）;观察组用药后2min SPO2显著高于对照组（布托啡诺组P〈0.01,芬太尼组P〈0.05）;清醒后VAS评分及丙泊酚总量观察组显著低于对照组（P〈0.05）。结论：丙泊酚用于人工流产术复合布托啡诺或芬太尼可以减少丙泊酚总量,能有效抑制或减轻术后宫缩痛,且不影响清醒质量,布托啡诺呼吸抑制作用较芬太尼更加轻微,还能抑制术后恶心、呕吐等不良反应。

Proptofol Combined with Butorphanol and Fentanyl for Induced Abortion Anesthesia

Objective： To observe the clinical analgesia effect and safety of proptofol, combined with butorphanol and fentanyl for induced abortion anesthesia. Method： 60 cases painless induced abortion patients were randomly divided into three groups, each having 20 cases. Group A and group B were slowly administered with 0.04mg/kg butorphanol and 0.002mg/kg proptofol within 30 seconds at normal speed, with the addition of 1mg/kg of proptofol in case of intraoperative of proptofol were recorded at 2minutes before and after the body movement. MAP, HR, SPO2 and the dosage use of the medicine, and at the end of the surgery as wen,VAS score of uterine contraction pain was also recorded at 5min,10min and 15min after the patients awakeness. Result： HT of the three groups has no distinctive difference. MAP of the observing group （ butorphanol and fentanyl ） at 2 minutes after the use of medicine was significantly lower than that of the control group （ P 〈 0.05 ） ; SPO2 of the observing group at 2 minutes after the use of medicine was significantly higher than that of the control group （ the group of butorphanol, P 〈 0.01, the group of fentanyl, P 〈 0.05 ） ; The observer group had a much more significant decrease than the control group in VAS score and the dosage of proptofol after awakening （ P 〈 0.05）. Conclusion： The use of proptofol combined with butorphanol and fentanyl for induced abortion anesthesia can reduce the dosage or proptofol and effectively inhibit the pain caused by uterine contraction without affecting sober quality. Compared with fentanyl, butorphanol has less effect on respiratory inhibition,and can inhibit the oostooerative adverse reactions such as nausea and vomiting.
