胺碘酮治疗心力衰竭并发室性心律失常112例分析

目的观察胺碘酮治疗心力衰竭并发室性心律失常的疗效及安全性。方法112例心力衰竭并发室性心律失常患者,经抗心力衰竭治疗病情稳定后,室性心律失常仍持续存在者应用胺碘酮0.2 g,每日3次,连服1周后改为0.2 g,每日2次连服1周,以后以0.2 g每日1次维持治疗12周。观察治疗前后心功能、室性心律失常改善、校正后Q-T间期和不良反应。结果治疗后112例患者中心功能提高Ⅰ级以上79.5%,室性心律失常总有效77.7%。治疗期间校正后Q-T间期延长,与治疗前比较有显著性差异(P<0.05),改用维持量12周后,校正后Q-T间期接近治疗前(P>0.05)。用药后出现肝功能异常17.9%,窦性心动过缓3.6%,经对症治疗或停药后恢复正常。结论胺碘酮治疗心力衰竭合并室性心律失常效果明显并且安全。

Analysis of effect of amiodarone on heart failure complicating ventricular arrhythmias for 112 cases

Objective To observe the effect and security of amiodarone on heart failure complicating ventricular arrhythmias.Methods One hundred and twelve cases with amiodarone on heart failure complicating ventricular arrhythmias who were gived the treatment of heart failure and the pathogenetic condition were stable but ventricular arrhythmias were still to remain,were gived amiodarone for 0.2 g,three times a day for one week,then changed the dose to 0.2 g,twice a day for one week,after that they were gived it for 0.2 g,once aday for 12 weeks to maintain therapy.Heart function, amelioration of ventricular arrhythmias,Q-T interval after remedy and adverse effect were observed pre-and post-treatment.Results In 112 cases,the rate of heart function increasesd aboveⅠstage was 79.5%,the total effective rate of ventricular arrhythmias was 77.7% and Q-T interval was prolonged post-treatment.There was significantly difference pre-and post-treatment(P<0.05).After treatment for 12 weeks with maintenance dose,Q-T interval was to approach the value of pre-treatment.The incidence of liver disfunction and sinus bradycardia were 17.9% and 3.6%respectively.They all recovered to normal after symptomatic treatment or discontinuation.Conclusion It is saft and effective of amiodarone to treat heart failure complicating ventricular arrhythmias.
