慢性丙型肝炎患者血清体液免疫指标的改变及其临床意义

目的:探讨慢性丙型肝炎(CHC)患者主要血清体液免疫指标的改变及其临床意义。方法:比较32例CHC患者和30例对照者的血清免疫球蛋白、循环免疫复合物(CIC)和补体水平。结果:CHC组IgG和IgM含量分别为17．2±3．68g／L和2．04±0．83g／L,显著高于正常对照组(P<0．01)。IgA含量略有升高,但和对照组相比无显著差异。C3、C4的含量分别为0．98±0．3g／L和0．19±0．075 g／L,低于正常对照组(P<0．01)。CHC组和对照组CIC含量分别为24．15±6．37mg／L和16．48±5．91mg／L,两组有显著差异(P< 0．01)。CHC组总蛋白和球蛋白的含量分别为77．2±8．16 g／L和35．0±5．19 g／L,高于正常对照组(P<0．05);白球比为1．23±0．26, 低于正常对照组(P<0．01)。结论:HCV慢性感染的病例,机体的体液免疫反应发生改变,动态监测HCV患者血清中的免疫球蛋白、补体及其CIC指标有助于疾病的诊断、治疗和预后判定。

Change of of Humoral Immunity Indexes in Patients with Chronic Hepatitis C and Its Clinical Impaication

Objective: To explore change of humoral immunity indexes in chronic hepatitis C patients (CHC) and its clinical implication. Methods: The serum immunoglobulin(IgG, IgM and IgA), complements (C3 and C4) and circulation immunity complex(CIC) were compared in 32 CHC and cantrols. Results: The results showed that the levels of IgG , IgM,and CIC in CHC group were significantly higher than thote of the control group(P<0.01). However, levels of C3 and C4 were lower than those of the control group (P<0.01). Furthermore, with the increases of total protein and globulin, the ratio of albumin to globulin decreased significantly in CHC group(P< 0. 01). Conclusion:These results suggested that humoral immunity responses should be impaired specially because of the liver disfunction after chronic hepatitis C virus infection and the dynamic surveillance on immunoglobulin, complements and CIC series could be beneficial to diagnosis and treatment of hepatitis C.
