甲状腺癌颈部淋巴结转移的超声特征

目的探讨甲状腺癌颈部淋巴结转移的超声特征。方法对96例甲状腺癌患者134个颈部肿大淋巴结行超声检查,并记录其超声特征。结果96例134个颈部肿大淋巴结中病理证实88个为转移性淋巴结肿大,46个为淋巴结反应性增生。超声检查显示:(1)转移性淋巴结中74个(74/88,84.1%)呈圆形或类圆形,其中16个(16/88,18.2%)呈融合状;(2)大多数转移性淋巴结表现为边界不清(76/88,86.4%)和内部回声不均(71/88,80.7%),其中淋巴结内见细小钙化灶50个(50/88,56.8%),淋巴结内见液化10个(10/88,11.4%);(3)大多数转移性淋巴结表现为血供丰富(42/56,75.0%)。结论甲状腺癌颈部转移性淋巴结具有特征性超声表现,多个指标综合判断可提高超声对甲状腺癌颈部淋巴结转移的检出率。

Sonographic features of cervical lymph node metastasis in thyroid carcinoma

Objective To evaluate the sonographic features of cervical lymph node metastasis in patients with thyroid carcinomas. Methods One hundred and thirty-four cervical lymph nodes in 96 patients with thyroid carcinomas were examined on ultrasound and their sonographic features were recorded. Results Pathological examination showed that 88 lymph nodes were metastatic and forty-six were reactive hyperplasia. The ultrasonic features were as follows: 1. In metastic lymphnodes, 74(74/88,84.1%) were round or approximately round and the inner echogeniety was heterogeneous in 16(16/88,18.2%); 2. 76(76/88,86.4%) were ill-defined, and 80.7% (71/88) showed heterogeneous echogeniety; 50(50/88,56.8%) showed internal micro-calcification and 10(10/88,11.4%) were liquidized inside; 3.Fourty-two(42/56,75.0%) revealed abundant blood supply. Conclusions The sonographic features of metastatic cervical lymph nodes from thyroid carcinoma were characteristic. Combination of multiple features will improve the correct diagnosis of cervical lymphnode metastasis.
