肝脏移植术后患者医嘱依从性调查与对策

目的观察肝脏移植患者出院后对医嘱依从性的状况以及医嘱依从性对移植肝功能的影响。方法通过对86例肝脏移植术后患者的随访,记录其性别、年龄、职业、文化程度、肝病类型、是否按医嘱要求服药、复查等情况及不依从医嘱的原因。结果肝脏移植术后患者对医嘱的依从率为41.86%,依从率低的主要原因有药费贵且需长期服用,其次是交通不便、家属的支持督导不力和心理疾病。依从性好的患者总体一年成活率可达86%,而依从性差的为54%,经χ2检验,医嘱的依从性好对疗效影响显著（P〈0.01）。结论提高患者对医嘱的依从性,可以保证移植肝的功能,提高患者的远期存活率。

Liver transplantation in patients with medical advice after investigation and compliance measures

Objective To observe the liver transplant patients after discharge status of compliance on the doctors’ advice and medical advice compliance function of the transplanted liver.Methods Their sex,age,occupation,education,type of liver disease,whether or not required by your doctor medication review,the reasons for non-compliance to medical advice were required and recorded in 86 cases performed liver transplantation on patients.Results On medical advice after liver transplantation in patients with 41.86% compliance rate,and the main reasons you need long-term use of drugs,followed by inconvenient transportation,supervision and support of family mental illness.Good patient compliance year overall survival rate was 86%,and poor compliance was 54%,and there was the significant difference（P0.01.） Conclusion Improve compliance of patients to medical advice can ensure graft function and improve long term survival of patients.
