水冷式双极射频纤维环成形术治疗椎间盘源性腰痛

摘    要:目的 观察水冷式双极射频纤维环成形术对椎间盘源性腰痛的治疗效果。方法 选取40例经椎间盘造影确诊为椎间盘源性腰痛、保守治疗无效的患者,采用水冷式双极射频纤维环成形术治疗,术后随访12个月,对治疗前和治疗后1w、1m、3m、6m、12m的VAS评分、Oswestry功能评分进行比较。 结果 术后患者疼痛评分明显降低,腰椎功能状态优良率术后1w 为75%(30/40)、术后1m、3m、6m、12m分别为82.5% (33/40)、80% (32/40)、77.5%(31/40)和75% (30/40)。术后1w、1m、3m、6m、12m的VAS和ODI评分明显低于术前(P?0.01)。所有病例未见严重并发症发生。结论 水冷式双极射频纤维环成形术治疗椎间盘源性腰痛效果良好,操作简单、安全,是椎间盘源性腰痛较为理想的微创介入治疗方法。

Abstract:【Abstract】 Objective:To observe the effect of water-cooled bipolar radiofrequency annuloplasty (WCBRA) on discogenic low back pain (DLBP). Methods:Forty patients,diagnosed by discography,undergoing DLBP were treated with WCBRA. All patients were followed up for 12 months. The Visual Analog Scale(VAS), Oswestry Disability Index(ODI) scores were evaluated and recorded on the 7th postoperative day and at the end of 1-month,3-month,6-month,12-month follow-up respectively. Results:The excellent rates of treatment on the 7th postoperative day and at the end of 1-month,3-month,6-month,12-month follow-up were 75%(30/40) ,82.5% (33/40) ,80.0% (32/40),77.5% (31/40),75% (30/40) respectively. The VAS and ODI scores at all postoperative time points were significantly lower than those before operation(P?0.01). No serious complication occurred. Conclusion:The WCBRA could provide better pain relief,improvement of lumbar function and fewer side effects. Therefore,it could be the choice for treating DLBP.
