面神经管膝状神经窝扩大:一种面神经管骨折的CT新征象

目的 探讨CT显示膝状神经窝扩大在面神经管骨折诊断中的价值。方法 对30例面神经管骨折患者行轴面和冠状面CT成像，以术中观察结果为准，评价膝状神经窝扩大与面神经管膝状神经窝骨折的相关性，比较轴面和冠状面图像对面神经管膝状神经窝扩大的显示情况。结果 术中发现面神经管膝状神经窝骨折30例，术前CT检查显示膝状神经窝骨折18例，术中发现膝状神经节肿胀30例，CT图像显示膝状神经窝扩大28例。CT检查及术中均显示膝状神经窝骨折、扩大18例，CT检查仅显示膝状神经窝扩大，未显示骨折12例。结论 外伤性面神经麻痹患者CT检查不能显示骨折线时，膝状神经窝扩大是诊断膝状神经窝骨折的重要依据。

The enlargement of geniculate fossa of facial nerve canal:a new CT finding of facial nerve canal fracture

Objective To discuss the value of enlargement of genieulate fossa of facial nerve canal in the diagnosis of facial nerve canal fracture. Methods Thirty patients with facial nerve canal fracture underwent axial and coronal CT scan. The correlation between the fracture and the enlargement of geniculate fossa of facial nerve canal was analyzed. The ability of showing the fracture and enlargement of geniculate fossa of facial nerve canal in axial and coronal imaging were compared. Results Fracture of geniculate fossa of facial nerve canal was found in the operation in 30 patients, while the fracture was detected in CT in 18 patients. Enlargement of geniculate ganglion of facial nerve was detected in 30 patients in the operation, while the enlargement of fossa was found in CT in 28 cases. Enlargement and fracture of geniculate fossa of facial nerve canal were both detected in CT images in 18 patients. Only the enlargement of geniculate fossa of facial nerve canal was shown in 12 patients in CT. Conclusion Enlargement of geniculate fossa of facial nerve canal was a useful finding in the diagnosis of fracture of geniculate fossa in patients with facial paralysis, even no fracture line was shown on CT images.
