女性压力性尿失禁尿道下吊带术后尿道壁的力学分析

摘    要:目的 测最雌性尿道壁和聚丙烯网片的力学特性,分析尿道下吊带手术治疗女性压力性尿失禁的力学机制.方法 对49例雌性猪尿道壁组织条和27例聚丙烯网片试件进行单轴拉伸试验测量力学特性.结果 雌性猪尿道壁的应力应变曲线呈抛物线,弹性模量与应力的平方根、应变均成正比,由极限应力和极限应变计算最大值弹性模量分别为:0.806 MPa和0.818 MPa;聚丙烯网片的弹性模最为(6.977±1.095)MPa.聚丙烯网片的极限应力显著高于尿道壁组织而极限应变显著低于后者(P<0.01).计算聚丙烯网片尿道下植入术可降低压力性尿失禁(stress urinary incontinence,SUI)患者尿道壁的变形最为术前的1/3.结论 聚丙烯网片特殊的力学性能,在施行尿道下吊带术后,可增强对尿道的支持,改善腹压增加时尿道的控尿能力.
