液基超薄宫颈细胞涂片9059例临床分析

目的:对液基超薄技术(Thinprep cytologic test,TCT)和TBS系统(The Bethesda system)在宫颈病变诊断中的临床应用价值进行评价。方法:对2005年4月~2006年12月在妇产科门诊就诊的9059例患者行TCT检查和TBS细胞学分类诊断,对TCT检查发现异常的患者进行阴道镜病理检查和HPV-DNA检测。结果:在9059例TCT检查涂片中,检出异常涂片478例(5.28%),其中ASCUS 402例(4.44%)、LSIL 43例(0.47%),HSIL 12例(0.13%),鳞癌10例(0.11%),腺癌1例(0.01%),另有HPV 109例(1.2%)。TCT与阴道镜下病理结果符合率:ASCUS为86.27%、LSIL为76.74%、HSIL为91.67%,鳞癌、腺癌均为100%,TCT检查与HPV-DNA检测符合率为64.22%。结论:TCT技术和TBS系统应作为无症状的健康妇女宫颈癌筛查的主要手段。

Clinical analysis of 9059 cervical smear using thinprep cytologic test and the Bethesda system

Objective:To evaluate the clinical value of thinprep cytologic test(TCT)and the Bethesda system(TBS)for the diagnostic of early ceryical diseases.Methods:A total of 9059 patients in our hospital had been examined by TCT and TBS from April 2005 to December 2006.The abnormal cases which had been selected by TCT were detected by colposcopical pathological examination and HPV-DNA testing.Results:We found 478(5.28%)abnormal smears out of 9059cases,in which atypical squamous cell of undetermined significance(ASCUS)402 cases(4.44%),low grade squamous intraepithelial lesion(LSIL)43 cases(0.47%),high grade squamous intraepithelial lesion(HSIL)12(0.13%),squamous cell carcinoma 10 case(0.11%),HPV infection 109 cases(1.2%),The rates of coordiance with pathological results of colposcopy ASCUS were 86.27%、LSIL were 76.74%、HSIL were 91.67%,and HPV-DNA were 64.22% respectively.Conclusion:TCT and TBS,may be a main method to screen and diagnose the precancerosis of cervix.
