缺血预处理对瓣膜置换术患者机体免疫功能的影响

摘    要:目的:探讨在体外循环过程中运用缺血预处理对瓣膜置换术患者机体免疫功能的影响。方法:24例瓣膜置换手术患者,分为对照组(10例)和预处理组(14例),观察两组患者免疫球蛋白IgG、IgM和补体C3、C4及IL-2的变化。结果:两组患者IgG、IgM和C3、C4、IL-2水平均在术后1d较术前明显降低,IgG、IgM在术后3d有所恢复,术后5d恢复正常,C3术后3d恢复正常。预处理组C4和IL-2在术后3d即恢复至术前水平,较对照组明显提前。结论:缺血预处理能促进瓣膜置换术患者机体细胞免疫功能的恢复,而对机体体液免疫的影响则可能通过对补体系统调节而实现。

Abstract:Objective To observe the effects of ischemic preconditioning(IPC)on the systematic immunologic function in the patients underwent the cardiac valve replacement operation. Methods Twenty-four patients needing the cardiac valve replacement operation were divided into the controlled group(n=10) and IPC group(n=14)). Venous blood samples were drawn at the 1, 3 and 5 d after surgery for the determination of plasma IgG, IgM, C3, C4 and IL-2. Results The values of all the items were significantly decreased at 1 d after surgery, and started to return to the normal values at 5 d after surgery in controlled group. IPC made the higher values of all the items get off to some extent and sooner turn back to the normal values, being in advance to 3 d after surgery. Conclusion IPC can advance the recovery of cellular immunity and humoral immunity, of which the latter is probably regulatee by complement route.
