Tei指数在急诊呼吸困难病因鉴别中的应用价值初探

目的初步探讨Tei指数在急诊呼吸困难病因鉴别中的应用价值。方法超声心动图测算161例急诊呼吸困难患者Tei指数，并与左室射血分数（LVEF）及E／A值进行相关性分析。以Tei指数0．47为临界值，计算其鉴别心源性和非心源性呼吸困难的敏感性及特异性。结果心源性呼吸困难组Tei指数显著高于非心源性呼困难组，Tei指数与LVEF及E／A值呈显著负相关（P〈0．01）。以Tei指数0．47为临界值，诊断心源性呼吸困难的敏感性为80．00％，特异性为69．01％。结论本研究提示Tei指数是反映心功能的有效指标，但其用于急诊呼吸困难病因鉴别的参纠懈限。

Exploration of its practical value of Tei index in differentiating etiology of dyspnea in emergency room

Objective To explore its practical value of Tei index in patients with different etiology of dyspnea in emergency room. Methods Tei indexes of 161 cases with dyspnea were measured in the emergency room. It was also analyzed on whether Tei indexes were correlated with LVEF and E/A respectively. Sensitivity＇ and specificity in differentiating etiology of dyspnea were calculated at Tei index based on the threshold of 0.47. Results Tei indexes in cardiogenic dyspnea group were significantly higher than those in noncardiogenic dyspnea group （P〈0.01）, and they were also negatively correlated with LVEF and E/A respectively （P〈O.01）. The sensitivity and specificity in diagnosing cardiogenic dyspnea at Tei index 0.47 were 80.00% and 69.01% respectively. Conclusion The study indicated that Tel index was a valuable parameter on cardiac insufficiency, yet it was deficient in differentiating etiology of dyspnea.
