Immunohistochemical study on metastatic carcinomas of the suprac-lavicular lymphnode

One hundred and seventy-five cases of supraclavicular lymphnodc with mctastatic carcinomas of unknown primary sites were examined by immunohistochemical localization. A panel of antibodies including rabbit -anti ke-ratin (EK.), thyroglobulin (TG)and monoclonal antibodies to gastric carcinoma (MG7) and hepatocellular carcinoma ( HAb-20) were used. The results showed that 67 of 175 cases were MG7 positive (38.3% )- among which 57.0% and 27-8% positive in adenocarcinomas and squamous carcinomas, respectively, all of undifferenliated carcinomas were negative. EK staining for MG7 negative cases showed 84.0% positive in squamous carcinomas, 60.0% positive in adenocarcinomas and 25.6% positive in undilferentiated carcinomas. Six of 8 cases of papillary carcinoma were positive by TG and 10 of 23 cases of adenocarcinoma were positive by HAb-20 (43.5% ). The results revealled that immunohistochemical localization with several markers was useful to delect the histological types of metastatic carcinoma in lymphnodes. It is of practical value to find primary sites in clinical diagnosis.
