CHF患者检测血浆BNP和hs-CRP的临床意义

目的探讨慢性心力衰竭（CHF）患者血浆脑钠肽（BNP）和超敏C-反应蛋白（hs—CRP）测定的临床价值。方法选择65例CHF患者（NYHA分级Ⅲ～Ⅳ级）作为CHF组，另选取33例健康体检者作为对照组，分别对治疗前及心功能改善至Ⅰ～Ⅱ级后的CHF患者及对照组进行血浆BNP和hs—CRP水平的测定。结果CHF组治疗前后血浆BNP和hs—CRP水平均显著高于对照组（P〈0．01）；CHF组在治疗后血浆BNP和hs—CRP水平均较治疗前显著降低（P〈0．01）；CHF心功能Ⅳ级组治疗前后血浆BNP和hs—CRP水平均显著高于心功能Ⅲ级组（P〈0．01）。结论血浆BNP和hs—CRP水平与CHF患者心功能不全严重程度密切相关，动态观察血浆BNP和hs—CRP水平的变化对判断病情严重程度及评价疗效有一定的临床价值。

Detection and its clinical significance of plasma BNP and high sensitivity C-reactive protein in patients with chronic heart failure

Objective To explore the clinical value of plasma BNP and high sensitivity C-reactive protein(hs-CRP) detected in chronic heart failure(CHF).Methods Sixty-five patients with CHF(NYHA class Ⅲ to Ⅳ)(CHF group) and 33 healthy cases(control group) were selected.The plasma BNP and hs-CRP were detected in CHF patients before treatment and when the heart function was improved to class ⅠtoⅡ and in control group.Results The levels of plasma BNP and hs-CRP in CHF patients before and after treatment were significantly higher than those of the control(P < 0.01).Compared with before treatment,the plasma BNP and hs-CRP in CHF patients decreased obviously after treatment(P < 0.01).The levels of plasma BNP and hs-CRP in heart function class Ⅳgroup were significantly higher than those of class Ⅲgroup before and after treatment(P<0.01).Conclusion The levels of plasma BNP and hs-CRP in CHF patients are correlated with severity of cardiac dysfunction.Moving observation of plasma BNP and hs-CRP have some clinical value for judging the severity and assessing the therapeutic effects.
