MIPPO技术治疗胫骨远端骨折的疗效观察

摘    要:目的探讨微创经皮钢板内固定术(MIPPO)治疗胫骨远端骨折的临床疗效,并总结治疗经验。方法46例胫骨远端骨折患者采用微创经皮钢板内固定技术治疗,观察手术时间、术中失血量、切口愈合、骨折愈合时间和完全负重时间,并按照Johner—Wruhs临床功能评分标准对患者最终功能进行评价。结果全部病例均获随访,切口均I期愈合;平均手术时间71min;平均术中失血量95mL;平均骨折愈合时间15.6周;平均完全负重时间16-3周;无延迟愈合、骨不连及钢板断裂、松动、再骨折发生。按照Johner—Wruhs临床功能评分标准,功能优35例,良9例,中2例。结论微创经皮钢板内固定治疗胫骨远端骨折创伤小,恢复快,并发症少,符合美学要求,是治疗胫骨远端骨折安全、有效的方法。

Abstract:Objective To investigate the clinical effect and summarize experience of Minimally Invasive Percutaneous Plate Osteosynthesis(MIPPO) in the treatment of distal tibia fractures.Methods The operation duration,blood loss,wound healing,union of fracture and full weight bearing were observed in 46 cases suffered from distal tibia fractures undergoing MIPPO.The final functional results were evaluated according to Johner&Wruhs criteria.Results All cases were followed up,and the incision healed by primary intention.The mean operation duration and blood loss were 71 minutes and 95 ml respectively.The mean union time of fracture was 15.6 weeks,while the mean time for full weight bearing was 16.3 weeks.Delayed union,nonunion,loosening and breakage of plate and refracture were not found in this group.Johner&Wruhs’ Evaluation showed 35 excellent,9 good and 2 fair cases.Conclusion Minimally invasive plate osteosynthesis is a safe and effective treatment for distal tibia fractures with the benefits of less damage,fast recovery,less complications and aesthetics requirement.
