热灌注化疗治疗恶性胸水30例临床观察

目的：观察体外高频热疗联合胸腔热灌注化疗治疗恶性胸水的疗效及不良反应。方法：恶性胸水60例,同期分为治疗组及对照组各30例。胸腔穿刺行闭式引流干净胸水后,治疗组给予43℃-45℃的生理盐水1000ml＋顺铂50mg/m2,以HGG2-102体腔热灌注治疗机灌注入胸腔,使进入胸腔的液体温度维持在41℃-43℃;灌注结束后,采用高频热疗机行胸腔局部热疗41℃-43℃,60分钟,每周热疗2次;对照组给予生理盐水1000ml＋顺铂50mg/m2胸腔内注入。两组均每周灌注1次,连续灌注3周,评定疗效。结果：治疗组有效率83.3%,KPS评分升高率63.3%;对照组有效率45.5%,KPS评分升高率31.8%。治疗组胸水控制率、KPS升高率均较对照组明显为优（P〈0.05）。两组不良反应无明显差异。结论：体外高频热疗联合胸腔热灌注化疗治疗恶性胸水疗效好,可以耐受。

Clinical study of hyperthermic perfusion chemotherapy combined with high-frequency hyperthermia in the treatment of malignant hydrothorax

Objective： To observe the efficacy and side effects of high-frequency hyperthermia combined with hyperthermic perfusion chemotherapy in the treatment of malignant hydrothorax. Methods： Total of 60 patients with malignant hydrothorax were divided into treatment group and control group,30 cases in each.In treatment group,after malignant hydrothorax was drained,normal saline 1000ml with cisplatin 50mg/m2 heated to 43-45℃ was perfused into pleural cavity,maintainning 41-43℃ with HGG2-102 body cavity perfusion therapy machine then pleural cavity was heated to 41-43℃ and keeping 60 minutes,twice a week.Patientsin control group was only given normal saline 1000ml and cisplatin 50mg/m2.For both groups,this therapeutic schedule was given once a week,for 3 weeks. Results： The effective rate was 83.3%,and KPS score increased rate was 63.3% in treatment group,while 45.5%,31.8% in control group（P0.05）.No significant difference between two gropus in side effects. Conclusion： High-frequency hyperthermia combined with hyperthermic perfusion chemotherapy in the treatment of malignant hydrothorax has good efficacy,and can be tolerated.
