尿激酶联合肝素钠治疗结核性胸膜炎的疗效观察

目的观察尿激酶联合肝素钠治疗结核性胸膜炎的疗效。方法将结核性粘连包裹性胸膜炎62例随机分为观察组（注射尿激酶和肝素钠组）及对照组（仅注射尿激酶）,其中,观察组32例,对照组30例。两组均给予抗结核、营养、支持、对症及口服糖皮质激素等常规治疗,并在B超定位下胸腔穿刺置入引流管,观察组加用尿激酶10万U和肝素钠4250U同时注入胸腔内,对照组仅注入尿激酶10万U,均在4～6小时后尽可能引流出胸水,两组进行比较。结果观察组总有效率96.875%,与对照组（63.33%）比较,差异有显著性（P〈0.01）,且观察组胸腔积液吸收时间及住院时间均短于对照组。结论胸腔内注入尿激酶和肝素钠能有效降低胸膜增厚和粘连,改善肺功能。

Urine kinase unites Heparin the sodium curative effect of treating tuberculosis pleurisy is observed

objective To observe the curative effect of combination treatment on pachypleuritis with urokinase and heparin sodium.Methods 62 of pleurisy divide into,observe group （inject group,kinase of urine and sodium of Heparin） and control group at random （inject urine kinase only） tuberculosis adhesion person who wrap up,among them,observe 32 groups,30 control groups.It offer two group last tuberculosis,nutrition,not support,to the ill and glucocorticoids oral,etc.treatments regular,orient lower thorax puncture put into drainage tube,observe group add,inject the thorax into with urine kinase 100,000U and Heparin sodium 4250U at the same time in Ultrasonography B,the control group only pours into urine kinase 100,000U,the drainage as much as possible produces water of chest after 4 hours,two groups compare.Results The total effectivenes is 96.875% in Observation group ,but 63.33% in control group.There is significante difference between them （P0.01）.Moreover ,the total length of stay and effusion absorption are all shorter in observation group.Conclusions Thorax pour into urine kinase and Heparin sodium can decrease pleura to be plump with adhesion effectively,improve the lung function.
