回肠膀胱输出道造口术前定位的临床研究

目的探讨回肠膀胱输出道造口术前定位对患者术后造口情况及生活质量的影响。方法作者医院2010-03/2014-02月因膀胱癌行全膀胱切除和回肠膀胱术（Bricker术）治疗的87例患者,随机分为观察组45例和对照组42例。观察组术前由造口治疗师、手术医生、护理人员、患者及家属共同参与造口定位,对照组由手术医生在术中进行定位造口。术后随访6个月,对患者的造口情况、生活质量进行记录并评估。结果观察组患者的造口并发症、对造口的适应性和生活质量明显优于对照组,两组比较差异具有统计学意义（P〈0.05）。结论输尿管回肠术前造口定位可有效减少造口术后并发症,增加患者对造口的适应性,明显提高患者术后生活质量。

The Clinical Research of Preoperative Ostomy Localization in Bricker Operation

Objective To discuss the condition and the influence on quality of life of preoperative ostomy localization in Bricker operation to patients.Methods Totally 87 patients with bladder cancer experienced radical cystectomy and Bricker operation in our hospital from march 2010 to february 2014,randomly divided into observation group（45 cases）and control group（42 cases）.In the control group,the colostomy therapists,doctors,nurses,patients and their families participated the preoperative ostomy localization together.In the observation group,the ostomy localization was implemented by doctors during operation.Followed up for 6 months,recorded and evaluated the ostomy situation and living quality of patients.Results In the control group,the complications of ostomy,the adaptability to ostomy and quality of life all superior to the observation group（P〈0.05）.Conclusion Preoperative ostomy localization in Bricker operation can effectively reduce the ostomy complications,increase the adaptability of colostomy and improve patients’ quality of life significantly.
