急性胰腺炎患者横结肠系膜腹膜下间隙受累与CT严重指数的相关性研究

目的:探讨急性胰腺炎(AP)累及横结肠系膜腹膜下间隙(SPST)与其CT严重指数(CTSI)的相关性。方法:回顾性分析58例符合Balthazar CT分级标准C级及以上的累及SPST的AP的MSCT资料。分别观察记录:AP患者的CTSI;横结肠系膜密度增高和增厚情况,SPST积液情况及其范围等。结果:①58例中,CTSI 4～5分者20例,6～7分者23例,8～10分者15例。SPST受累的CT表现为间隙内密度增高、条索影、积液、系膜增厚、系膜血管边缘模糊等;其中,SPST积液者21例。②AP患者的CTSI不同,SPST的纵向受累范围差异有统计学意义(P<0.01)。结论:AP时SPST受累情况与其CTSI具有一定的相关性。

Correlative study of subperitoneal space of transverse mesocolon involvement with CT severity index in acute pancreatitis

Objective： This study aimed to demonstrate the correlation between involvement of subperitoneal space of transverse mesocolon（SPST）and CT Severity Index（CTSI）in acute pancreatitis（AP）.Methods：MSCT of fifty-eight patients with AP were retrospectively reviewed.All the patients′ Balthazar′s CT classification was C grade or more,and the SPST was involved.Emphasis was placed on the following findings：the CTSI,the density and thickness of the SPST,fluid collection within the SPST and its scope.Results：Among the 58 patients,20 patients belong to the group which the CTSI was 4-5,23 patients belong to the group which the CTSI was 6-7,and 15 patients belong to the group which the CTSI was 8-10.The CT manifestations of SPST involvement include higher density,stripe shadow,thickness and fluid collection of the SPST.21 patients showed fluid collection in the SPST.The extent of the involvement of the SPST showed positive correlation with the CTSI in patients with acute pancreatitis（P〈0.01）.Conclusion：The results showed that the involvement of SPST could reflect the severity of AP to some extent.
