IHC和FISH检测乳腺癌HER2基因表达的比较研究

摘    要:目的探讨荧光原位杂交(FISH)与免疫组织化学(IHC)检测HER2基因状态的结果的一致性及HER2表达与其淋巴结转移的关系及意义。方法采用IHC技术检测68例乳腺浸润性导管癌HER2基因的表达,同时进行FISH检测。结果HER2表达(+)的28例标本中,其中27例经FISH检测无扩增,1例有扩增;HER2表达(++)的12例标本中,7例经FISH检测无扩增,2例有扩增,另3例为可疑;HER2表达(+++)的28例标本中,经FISH检测均扩增;FISH检测有扩增的30例标本中,26例发生了淋巴结转移;可疑的3例中有2例有淋巴结转移;无扩增的34例标本中只有6例发生了淋巴结转移。结论当HER2表达为(+)或(+++)时。IHC和FISH检测的一致性较高,而当HER2表达为(++)时,必须进行FISH检测;HER2的扩增与淋巴结的转移关系密切,呈正相关(P〈0.05)。

A Comparative Study on HER-2 Expression by Immunohistochemistry and Fluorescence in Situ Hybridization in Breast Cancer Tissue

Abstract:Objective To explore the relationship between HER-2 gene expression and lymph node metastasis by Fluorescence in Situ Hybridization ( FISH), and to compare these results with those obtained by immunohistocbemistry (IHC). Methods IHC and HSH were employed to determine the expression of HER-2. Results Among 28 samples with HER-2 expression score ( + ) by IHC, 27 were negative for HER-2 expression by FISH and only one was positive. Among 12 samples with score ( + + ) by IHC, 7 were negative and 2 were positive by HSH, remaining 3 uncertain. All of the 28 samples with HER-2 expression score ( + + + ) by IHC were also positive by FISH. In addition, 26 of the 30 cases with positive HER-2 expression by FISH were identified with lymphatic metastasis; 2 of the 3 uncertain cases were identified with lymph node metastasis; however, only 6 of the 34 cases with negative HER-2 expression by FISH were identified with lymph node metastasis. Conc/uslon A high consistent results are showed by IHC and IHC when HER-2 expression score is ( + ) or ( + + + ). However, when the HER-2 expression score is ( + + ), the results of these two methods are different, so HSH must be employed. Additionally, there is a positive correlation between HER-2 expression and lymph node metastasis (p 〈0. 05).
