慢性肺心病急性加重诱因及细菌感染临床分析

摘    要:目的探讨肺心病急性加重的诱发因素,分析细菌学感染特征。方法选择呼吸科资料完整的肺心病急性加重期患者85例,按临床资料进行诱因分析,并对痰培养结果进行初步分析。结果以感染为诱因占66%,其他诱发因素包括劳累、气胸、镇静剂使用等。城镇居民和农村居民感染率分别为64%和68%,无显著差异。冬春季节和夏秋季节感染率分别为76%和50%(P<0.05)。感染患者细菌培养结果以革兰氏阴性杆菌为主,占总数的50%。结论感染是诱发肺原性心脏病急性加重的重要因素,冬春季节是感染高发季节,革兰氏阴性杆菌感染比例较高。

Abstract:AIM To investigate the causative factors and characteristics of bacterial infection in acute exacerbation of chronic pulmonary heart disease. METHODS The clinical data of 85 patients with acute exacerbation of chronic pulmonary heart disease were studied and the characteristics of the bacterial infection in the sputum culture were analyzed.RESULTS Among the subjects under study,65.9%(56/85) of the causative factors of acute exacerbation was related to the bacterial infection.The infection rate of winter and spring was higher than that in summer and autumn(P<0.05).No significant difference was found related to the living areas of the patients.The dominant bacteria was gram-negative(G-) bacilli(50%)and Gram positive(G ) bacteria accounted for 31%.CONCLUSION Bacterial infection is the major causative factor for patients with acute exacerbation of chronic pulmonary heart disease and Gram-negative bacteria is the dominant pathogenic bacteria
