脑干听觉诱发电位及基底池形态变化对重型颅脑损伤患者预后判断的意义

摘    要:目的:研究脑干听觉诱发电位(BAEP)及基底池CT改变对判断重型颅脑损伤患者预后的价值。方法:对50例重型颅脑损伤患者进行BAEP测定及CT检测观察基底池形态变化,分析它们与格拉斯哥计分(GCS)及格拉斯哥预后计分(GOS)之间的关系。结果:随着病情的加重,BAEP和脑基底池形态的异常程度加重。重型颅脑损伤患者病情恶化后,BAEP的主要变化是按Ⅴ-Ⅲ-Ⅰ波的顺序逐渐消失,其次是各波潜伏期的延长及峰间潜伏期的延长;基底池的变化规律是环池受压→四叠体受压→环池及四叠体池闭塞→脑干变形。结论:BAEP检测及基底池形态的变化可评估重型颅脑损伤患者的脑干功能并为判断预后提供依据。

Abstract:Objective:To study the value of brainstem auditory evoked potential (BAEP) and changes of basal cistern in CT scanning for prognosis of severe head injury patients.Methods:The BAEP of 50 severe head injury patients were detected and the morphological changes of basal cistern were abserved by CT scan.The relationships between BAEP,CT changes of basal cistern and Glasgow Come Score (GCS),Glasgow Outcome Score (GOS) were analyzed.Results:Along with aggratating of disease,the abnormality of BAEP and morphological changes of basal cistern also aggratated. When the illness got worse,the changes of BAEP were that waves disappeared as Ⅴ-Ⅲ-Ⅰ in sequence and then the latencies of the waves and the intervals prolonged. The changes of the basal cistern were that in the following:compression of the ambient cistern-compression of the quadriyenunal cistern-obliteration of cisterna ambiens and quadrigeminal cistern-deforming of brainstem. Conclusions:The changes of BAEP and basal cistem were considered to be valuable in evaluating the function of brainstem and prognosis of severe head injury patients.
