正畸-牙周手术-修复联合治疗保存残根的临床效果研究

目的探讨正畸-牙周手术-修复联合疗法在保存残根治疗中的临床应用效果。方法选取2008年12月～2011年12月在我院进行残根牙治疗的86例患者作为对象，将人选对象分为研究组和对照组各43例。研究组患者采用正畸-牙周手术-修复联合疗法进行治疗，对照组患者则仅采用残牙修复疗法，比较和分析两组患者的临床治疗效果和并发症发生情况。结果研究组患者优良比率和总有效率分别为60．47％和95．35％．均显著高于对照组的39．53％和72．09％，差异有统计学意义（X^2=3．76、3．88、3．88；P〈0．05）；与对照组相比，研究组患者Ⅰ度、Ⅱ度、Ⅲ度冠桥松动比率均明显减少，差异均有统计学意义（X^2=3．04、3．22、6．08；P〈0．05）。结论对于折断面位于龈下的残根，当处于患牙适应证范围内时，采用正畸-牙周手术-修复联合治疗技术,不仅可以有效地保存天然残根，降低患牙并发症，又可最大程度上维持修复体美观性，在临床上具有较好的应用价值，适于进一步推广应用。

Clinical effect of orthodontics-periodontal surgery-repair combination therapy in saving dental residual roots

Objective To investigate the clinical effect of orthodontics-periodontal surgery-repair combination therapy in saving residual roots. Methods A total of 86 patients with dental residual roots from December 2008 to December 2011 in our hospital were selected as clinical research objects, all patients were divided into study group and control group, with 43 cases in each group. Patients in study group were given orthodontics-periodontal surgery-repair combi- nation therapy, and patients in control group were only given residual dental restoration therapy, clinical effect and complications of two groups were analyzed and compared. Results The excellent rate and total effective rate of study group （60.47% and 95.35%） were significantly higher than those of control group （39.53% and 72.09%） , the differ ence between the two groups were statistically significant （X^2=3.76, 3.88, 3.88,P〈0.05）. Compared with control group, the patients with Ⅰ , Ⅱ, Ⅲ degree of crown bridge loosening in study group were significantly reduced, the difference was statistically significant （X^2=3.04,3.22,6.08; P〈0.05）. Conclusion For subgingival residual roots, within the range of in- dications, the orthodontics-periodontal surgery-repair combination therapy can not only effectively preserve the natural residues, reduce the teeth complications, it also can restore aesthetics of restoration, and has good application value in clinical practice, suitable for further application.
