血液灌流联合血液透析抢救重度有机磷农药中毒的疗效观察

摘    要:目的探讨在药物治疗的基础上同时应用血液灌流联合血液透析抢救重度有机磷农药中毒的临床疗效。方法回顾性分析2008年1月~2013年1月我院及中国医科大学附属第一医院急诊科收治的重度有机磷农药中毒患者52例的临床资料。其中30例应用血液灌流联合血液透析同时治疗,设立为观察组;选择同期单纯应用药物治疗及血液灌流未应用血液透析治疗的22例重度有机磷农药中毒患者设立为对照组,比较两组的治愈率、阿托品用量、住院时间、中间综合征的发生率。结果观察组的治愈率明显高于对照组,差异有显著性(t=3.891,P〈 0.05)。观察组阿托品用量明显少于对照组,差异有显著性(t = 62.125,P〈 0.01)。观察组住院时间明显短于对照组,差异有显著性(t = 3.865,P〈 0.05)0观察组中间综合征发生率明显低于对照组,差异有显著性(χ2=4.125,P〈0.05)o结论在药物治疗的基础上同时应用血液灌流联合血液透析抢救重度有机磷农药中毒疗效确切,值得临床推广。

Abstract:Objective To investigate the efficacy of hemoperfusion combined with hemodialysis rescuing severeorganophosphorus pesticide poisoning clinical. Methods A retrospective analysis of 52 patients with the clinical datatreated by severe organophosphorus pesticide poisoning from January 2008 to January 2013 in our hospital andemergency department of the first affiliated hospital of china medical university. 30 patients were treated withhemoperfusion combined with hemodialysis treated at the same time, the establishment of the observation group,selected the corresponding period in the simple application of drug therapy and blood perfusion hemodialysis treatmentof 22 cases of severe organophosphorus pesticide poisoning patients set up for the control group, the two groups werecompared cure rate, the dosage of atropine, length of stay, intermediate syndrome incidence. Results The cure rate ofthe observation group was significantly higher than that in the control group, the difference was significant (χ2 = 3.891,P 〈0.05). The dosage of atropine observation group was significantly less than the control group, the difference wassignificant (t = 62.125, P 〈0.01). The observation group hospital stay was significantly shorter than the control group,the difference was significant (t = 3.865, P 〈 0.05). The observation group intermediate syndrome were significantlylower than the control group, the difference was significant (χ2 = 4.125, P 〈 0.05). Conclusion The application ofdrug therapy on the basis of hemoperfusion combined with hemodialysis rescue severe organophosphorus pesticidepoisoning is effect, worthy of promotion.
