防旋型股骨近端髓内钉与动力髋螺钉治疗股骨粗隆间骨折疗效比较

目的比较防旋型股骨近端髓内钉与动力髋螺钉（dynamichipscrew，DHS）内固定治疗股骨粗隆间骨折的疗效。方法我院2009年1月-2012年1月在我院行PFNA内固定治疗的股骨粗隆间骨折患者32例设立为观察组（Evans分型：Ⅰ型26例，Ⅱ型6例），选择同期行DHS内固定治疗的股骨粗隆间骨折患者30例设立为对照组（Evans分型：Ⅰ型22例，Ⅱ型8例），比较两组患者的手术时间、术中出血量、骨折愈合时间。结果观察组的手术时间明显短于对照组，差异有统计学意义（P〈0．05）；观察组术中出血量明显少于对照组，差异有统计学意义（P〈0．05）；观察组患者骨折愈合时间明显快于对照组，差异有统计学意义（P〈0．05）。观察组与对照组髋关节功能优良率比较，差异不显著（P〉0．05）。结论与DHS内固定比较，PFNA内固定治疗股骨粗隆间骨折固定可靠、并发症少、疗效确切，值得推广和应用。

Anti-rotation of proximal femoral nail and dynamic hip screw in treat- ment of intertrochanteric fractures

Objective To compare the efficacy of anti-rotation of proximal femoral nail and DHS internal fixation of intertrochanteric fracture. Methods In our hospital from January 2009 to January 2012,PFNA within fixed treatment femur intertrochanteric fracture patients established in 32 cases in the observation group, selected over the same period DHS fixation of intertrochanteric fractures of 30 patients for the control group,the operative time,blood loss,fracture healing time were compared between the two groups. Results The operation time of the observation group was signifi cantly shorter than the control group （P〈0.05）; blood loss of observation group was significantly less than the control group （P〈0.05）; The patients fracture healing time was significantly faster than the control group-（P 〈 0.05 ）. Observation group compared with the control group,hip function excellent rate had no differenee（P 〉 0.05）. Conclusion Compared with DHS,the PFNA within fixation of femoral intertrochanteric fracture fixation is reliable, has fewer complieations and good efficaey, and it is worthy of promotion and application.
