血尿酸水平与脑卒中患者心功能的相关性研究

目的探讨血尿酸水平与脑卒中患者心功能的相关性。方法选取2010年6月-2013年3月在我院神经内科住院的脑卒中患者183例，同期健康体检者78例，分别测血尿酸（UA）、B型脑钠肽（BNP）和行心脏超声心动图检查。结果脑卒中组UA浓度较正常对照组显著增高（聘6．37，P〈0．05）；脑卒中组的各亚组患者之间UA浓度不全相等（F=17．30，P〈0．05），其中脑血栓形成组UA浓度分别较腔隙性脑梗死组、短暂性脑缺血发作组及脑栓塞组脑卒中组显著增高（分别t=8．24，P〈0．05；t=6．43，P〈0．05；t=7．17，P〈0．05）。脑卒中组UA值与BNP值正相关（r=0．210。P〈0．05），与左心室舒张末内径正相关（r=0．223，P〈0．05），与左心室收缩末内径正相关（r=0．209，P〈0．05）；与左心室射血分数负相关（r=-0．386，P〈0．05）。结论脑卒中患者血尿酸水平与BNP具有较好的一致性，血尿酸水平与心功能同样具有良好的相关性。

The correlation of serum uric acid level and cardiac function in patients with cerebral infarction

Objective To detect the relationship between serum uric acid and the cardiac function in patients with cere- bral infarction. Methods A total of 183 cases of patients with cerebral infarction were chosen as observation group, 78 cases of healthy people in the same stage were chosen as control group. The levels of serum uric aid andbrain atriuret- ic peptide in all chosen subjects were determined andcardiac functions were evaluated by echocardiography. Results The levels of UA was significantly higher in observation group than that in control group （F=6.37,P〈0.05）. Further- more,the patients with cerebral infarction had significantly different UA levels between subgroups （F=17.30,P〈0.05）. In additon, patients with cerebral thrombosis had significantly different levels of serum UA ,when comparing with pa- tients with lacunar infarction and transient ischemic attack, as well as with cerebral embolism （t=8.24,P〈0.05;t=6.43, P〈0.05;t=7.17,P〈0.05,respecfively）. In patients with cerebral thrombosis,UA was independently associated with serum BNP level（r=0.210,P〈0.05）, as well as with left ventricular end diastolic diameter（r=0.223 ,P〈0.05）,left ven- tricular end systolic diameter（r=0.209 ,P〈0.05） and left ventricular ejection fraction （r=-0.386,P〈0.05）. Conclusion The level of serum UA is correlated with BNP, as well as with cardiac function.
