会厌瓣在额侧喉部分切除术喉功能重建中的应用

目的探讨会厌瓣在额侧喉癌和侵及额侧喉癌喉部分切除术喉功能重建中的应用。方法29例喉癌患者分别采用单纯会厌瓣、会厌瓣加一侧胸骨舌骨肌筋膜或双侧胸骨舌骨肌肌筋膜修复喉腔缺损。结果29例患者术后拔除气管套管的拔管率100％，轻度误咽2例，两个月内消失，言语功能基本正常，3、5年生存率分别为89．47％、78．57％。结论额侧喉癌和侵及额侧喉癌喉部分切除术应用会厌瓣和胸骨舌骨肌筋膜重建喉功能，具有取材方便、操作简单、气管套管拔管率高的特点，可提高患者术后的生活质量。

Application of frontolateral partial laryngectomy on laryngeal cancer treatment and reconstruction with epiglotiic flap

Objective To explore the application of both frontolateral and frontolateral involved laryngeal cancer on partial laryngectomy and reconstruction with epiglottic flap. Methods In 29 cases of laryngeal cancer, used respectively only epiglottic flap, or epiglottic flap united one or double side sternohyoid muscle fascia flap to repair laryngeal defect and reconstruction during operation. Results In 29 cases, the decannulation rate was 100%, 2 cases of deglutition disorder was disappeared within two months, the language ability basic was normal and the estimated 3- and 5-year survival rates were respectively 89.47% and 78.57%. Conclusion In frontolateral and frontolateral involved laryngeal cancer patients, during their partial laryngectomy, the application of epiglottic and sternohyoid muscle fascia flap to re- constructe laryngeal defect, it＇s feature were more convenient the material selected, simpler operative way, higher decannulation rate, and better patients＇ the quality of life after operation.
