影响胃癌预后的相关因素研究

目的研究影响胃癌术后预后情况的相关因素。方法回顾性分析我院2005年12月～2008年12月住院手术的86例胃癌患者的临床及随访资料，对各类资料进行统计分析。结果本次研究中1、3、5年的生存率为74．42％、58．14％、22．09％，生存时间为10～73个月，中位生存期为49．5个月；吸烟（X^2=10．66，P〈0．05）、临床分期（X^2=16．98，P〈0．05）、肿瘤大小（X^2=20．18，P〈0．05）、浸润程度（X^2=18．92，P〈0．05）对胃癌患者的1、3、5年生存率具有显著的影响；Cox结果表明临床分期为Ⅲ、Ⅳ期（RR=I．45，95％CI：1．28～2．96）、肿瘤≥5cm（RR=1．32．95％CI：1．20～2．79）、浸润程度侵及浆膜（RR=1．73，95％CI：1．37～3．06）是影响患者生存的独立危险因素。结论胃癌病情越重、肿瘤越大、浸润程度越大，胃癌手术患者的生存期越短。

Study on related factors affecting prognosis of gastric cancer

Objective To study the related factors affecting prognosis of gastric cancer after surgery. Methods Retro- spectively analyzed the clinical data and follow-up data of 86 patients with gastric cancer hospitalized in our hospital from December 2005 to December 2008, statistical analyzed all kinds of data. Results The survival rates of 1, 3, 5 years were 74.42%, 58.14%, 22.09% respectively, the survival time was 10-73 months, the median survival time was 49.5 months; Smoking （X^2=10.66, P 〈 0.05）,clinical stage （X2=16.98, P 〈 0.05）,tumor size （X^2=20.18, P 〈 0.05）, and degree of infiltration （X^2=18.92, P 〈 0.05） had significant impact on survival rate of 1, 3, 5 years; the results of Cox showed that the clinical stage of Ⅲ、Ⅳ（RR=1.45, 95% CI： 1.28-2.96）, tumor size≥5 cm（RR=l.32,95% CI： 1.20- 2.79）, infiltration invade serous membrane（RR=1.73, 95% CI： 1.37-3.06） were independent risk factors affecting survival. Conclusion The heavier illness of gastric cancer, greater of tumor size and greater of the degree of infiltra- tion,the shorter of survival time in patients with gastric cancer surgery.
