术中黏膜下注射与术后早期膀胱灌注吡喃阿霉素预防浅表膀胱癌复发疗效的比较

摘    要:目的:对比吡喃阿霉素(THP)术中黏膜下注射与术后早期膀胱灌注预防浅表膀胱癌膀胱部分切除术后复发的疗效。方法:73例患者随机分为A、B两组。A组于手术当天6小时内、术后第7天给予术后早期20mgTHP膀胱灌注;B组术中给与20mgTHP膀胱黏膜下注射。两组均术后第2周开始20mgTHP常规方案膀胱灌注,1次/周共8次,之后1次/月至1年,记录术后血尿情况和常规灌注不良反应。定期膀胱镜随访。结果:所有患者随访3年,术后第1、2和3年肿瘤无复发率A组为85.7%、80%和77.1%,B组为92.1%、81.6%和76.3%,两组间无明显差异(P〈0.05),两组患者术后平均血尿天数分别为(1.7±0.9)天和(6.2±1.5)天,两组间差异有统计学意义(P〈0.001)。所有患者术后未见明显全身药物不良反应,21例患者出现下尿路症状,对症处理后缓解,未影响治疗。结论:THP术中黏膜下注射和术后早期膀胱灌注显著降低浅表膀胱癌术后复发率,两者疗效差异无统计学意义。术后早期膀胱灌注方案膀胱损伤小,耐受性更高。
