从“瘀、毒、郁”探讨慢性萎缩性胃炎的病机

瘀毒致病是阐释慢性萎缩性胃炎(CAG)发生、发展的重要理论,认为瘀血、湿浊、湿热蕴结,久化瘀毒、浊毒,裹挟留滞,终成热毒,隐匿为害,导致本病迁延难愈。此外,焦虑、抑郁状态(郁)亦是CAG发生、发展的危险因素。探讨瘀、毒、郁的内涵以及三者在CAG中的致病机制,并通过辨析瘀与毒、郁与瘀、毒与郁之间的关系,认为“瘀、毒、郁”互结是CAG的核心病机,其中“郁”可能是促进瘀血、湿浊、湿热转为瘀毒、浊毒,进而化生热毒的病理要素。

Exploration on the Pathogenesis of Chronic Atrophic Gastritis from the Perspectives of Stasis,Toxins,and Constraints

The theory of“stasis and toxins leading to diseases”which is widely used in explaining the evolving process of chronic atrophic gastritis(CAG),holds that the long-term accumulation and binding of blood stasis,dampturbidity and damp-heat may evolve into stasis toxin and turbidity toxin,which can further develop into heat toxin.Due to the concealed conduction,the disease is protracted and refractory.Additionally,anxiety and depression(con?straints)are also the risk factors for the occurrence and development of CAG.By exploring the connotation of stasis,toxins and constraints and their pathogenic mechanism in CAG,and by distinguishing the relationships of stasistoxins,constraints-stasis,and toxins-constraints,it is considered that the binding of stasis,toxins and constraints should be the core pathogenesis of CAG,and“constraints”is the pathological factor to promote the development of stasis toxin and turbidity toxin from blood stasis,damp-turbidity and damp-heat,and then transform into heat toxin.
