摘    要:目的:探讨2型糖尿病(T2DM)患者血清MOTS-c水平与心脏功能不全的相关性。方法:选择2019年5月—2021年4月山东医学高等专科学校附属医院内分泌科住院治疗的T2DM患者123例,其中无并发症22例(对照组),伴心功能Ⅰ,Ⅱ,Ⅲ,Ⅳ级分别为28例、30例、26例、17例(心功能Ⅰ,Ⅱ,Ⅲ,Ⅳ级组)。采用问卷调查收集患者基本资料,包括性别、年龄、个人史(烟酒史)、既往史、家族史及近期用药情况等。所有受试对象测定身高、体质量、血压、空腹血糖(FPG)、总胆固醇(TC)、甘油三酯(TG)、低密度脂蛋白胆固醇(LDL-C)、高密度脂蛋白胆固醇(HDL-C)。采用酶联免疫吸附法(ELISA)测定所有受试对象空腹血清MOTS-c水平,并计算体质量指数(BMI)。结果:与对照组比较,心功能Ⅰ,Ⅱ,Ⅲ,Ⅳ级组患者血清MOTS-c水平均明显降低(P<0.01),且心功能Ⅲ级和Ⅳ级组患者血清MOTS-c水平低于心功能Ⅰ级和Ⅱ级组(P<0.05),心功能Ⅰ级组与Ⅱ级组、Ⅲ级组与Ⅳ级组组间比较,差异均无统计学意义。在糖尿病伴心功能不全患者中,血清MOTS-c与年龄、TC、TG、LDL-C呈显著正相关(P<0.05)。多元线性回归显示,TC,TG,LDL-C是血清MOTS-c的独立相关因素。结论:血清MOTS-c的低表达可能与糖尿病伴心功能不全的发生及发展相关,可能参与糖尿病患者的脂肪代谢。MOTS-c是糖尿病伴心功能不全的潜在保护因素,或将成为治疗糖尿病伴心功能不全的新靶点。

Abstract:Objective:The purpose of this study was to investigate the association between serum MOTS-c levels and cardiac insufficiency in patients with type 2 diabetes mellitus(T2DM).Methods:In this study,from May 2019 to April 2021,123 T2DM patients were hospitalized in the department of endocrinology of affiliated hospital of shandong medical college,and 22 were in the T2DM uncomplicated group(control group).There are 28 people,30 people,26 people,and 17 people in theⅠ,Ⅱ,Ⅲ,Ⅳgrade groups respectively(cardiac function groupsⅠ,Ⅱ,Ⅲ,andⅣ).The basic information of the patients was collected by questionnaire,including gender,age,personal history(smoking and alcohol history),past history,family history and recent medication,etc.All subjects were measured for height,weight,blood pressure,fasting blood glucose(FPG),total cholesterol(TC),triglyceride(TG),low density lipoprotein cholesterol(LDL-C),and high density lipoprotein cholesterol(HDL-C).Fasting serum MOTS-c levels of all subjects were determined by enzyme linked immunosorbent assay(ELISA),and body mass index(BMI)was calculated.Results:Compared with the control group,the serum MOTS-c level of patients with cardiac function in cardiac function groupsⅠ,Ⅱ,Ⅲ,andⅣwas significantly decreased(P<0.01),and the serum MOTS-c level of patients with cardiac function inⅢ,Ⅳgroups was lower than that inⅠ,Ⅱgroups(P<0.05).There was no difference betweenⅢ,Ⅳgroups and betweenⅠ,Ⅱgroups.In diabetic patients with cardiac dysfunction,serum MOTS-c was significantly positively correlated with age,TC,TG and LDL-C(P<0.05).Multiple linear regression showed that Total cholesterol(TC),triglyceride(TG)and LDL-C were independent related factors of serum MOTS-c.Conclusion:The low expression of serum MOTS-c may be related to the occurrence and development of diabetes with cardiac insufficiency,and may be involved in fat metabolism in patients with diabetes.MOTS-c is a potential protective factor of diabetes with cardiac insufficiency,and may become a new target for the treatment of diabetes with cardiac insufficiency.
