腹膜后局灶性Castleman病8例临床特点及治疗

[摘要] 目的 深化对腹膜后局灶性Castleman病(Castleman disease,CD)的认识，提高腹膜后局灶性CD的诊治水平。方法 回顾分析8例腹膜后局灶性CD患者的临床特征及诊疗情况，总结其临床特点及诊治策略。结果 8例患者中3例表现为腹痛，2例表现为副肿瘤性天疱疮，3例无症状；实验室检查多无异常；早期诊断困难，确诊有赖于组织病理学检查，病理学类型均为透明血管型；均行手术切除，1例术后复发，历经3次手术，最终死亡，其余病例均治愈，未见复发。结论 腹膜后局灶性CD多无特异症状，临床诊断困难，确诊有赖于组织活检病理检查，手术切除可望获得根治。

Clinical Features and Treatment of 8 Patients with Retroperitoneal Local Castleman Disease

Objective To deepen awareness of retroperitoneal local Castleman disease (Local Castleman disease, LCD), and to improve its diagnosis and management. Methods Clinical characteristics and treatment of eight cases of retroperitoneal LCD were retrospectively analyzed, and its clinical features and treatment strategies were reviewed. Results 3 cases of the 8 took a symptom of abdominal pain, 2 cases took a symptom of paraneoplastic pemphigus, and the other 3 cases took a symptom of asymptomatic. Laboratory examinations were almost normal. Early diagnosis was difficult to make, and the final diagnosis depended on the histopathology test, which showed hyaline-vascular type in all cases. All the 8 patients took surgical resections, among which one patient had recurrent diseases and finally died after three operations. Conclusion Most of retroperitoneal LCD had no specific symptom, and clinical diagnosis of them were hard to make, which need histopathology test to finally define it, and surgical resection was expected to be radical treatment.
