摘    要:目的对内科患者下呼吸道感染病原菌的构成及耐药情况进行分析,指导临床合理用药。方法收集2008年1~12月诊断为下呼吸道感染的内科住院患者的痰标本进行分离培养,并用全自动微生物鉴定和药敏分析仪对培养出的致病菌进行药敏试验,记录和统计试验结果。结果在526例痰标本中共分离出15种218株病原菌,其中9种主要病原菌的分布情况是:流感嗜血杆菌20株,副流感嗜血杆菌30株,大肠埃希菌12株,肺炎克雷伯菌40株,铜绿假单胞菌38株,金黄色葡萄球菌14株,肺炎链球菌10株,真菌22株,不动杆菌15株;耐药情况为:金黄色葡萄球菌对10种抗菌药物的总耐药率为61.0%,真菌对7种抗菌药物的耐药率为7.7%。结论下呼吸道感染患者的痛原菌种类较多,革兰阴性菌感染占大多数,大肠埃希菌和铜绿假单胞菌的耐药率较高。

Abstract:Objective To analyze sputum-cultivated pathogenic bacterium distribution and drug resistance ratio to the inpatients suffered from lower respiratory tract infection in our hospital, and to guide the rational drug administration in clinic.Methods To collect the inpatients' Sputum specimens from January 2008 to December 2008,conduct automatic microorganism identification and drug sensitivity test analysis system to process the specimens, obtain the isolated pathogenic bacterium progress drug sensitivity test, and take down the records and statistic test results.Results Among 526 sputum examples, 13 species or 218 strains pathogenic bacterium were isolated. And 9 of 13 species pathogenic bacterium distributed were as follow:there were 20 strains H.haemolyticus,30 strains H.parahaemolyticus,12 strains E. coli,40 strains K. pneumoniae,38 strains P.aeruginosa,14 strains S.aureus,10 strains S.pneumoniae,15 strains A.baumannii and 22 strains Fungus etc;the complexion of drug resistance suggested the total drug resistance of the S.aureus anti 10 kind antibiotic was 61.0% and fungus anti 7 kind antibiotic was 7.7%.Conclusion The species of pathogenic bacterium were various.Gram negative bacteria were the main pathogenic bacteria involved in respiratory tract infection. The drug resistance was high at E. coli and P.aeruginosa.
