左卡尼汀对血液透析患者心功能及血脂的影响

目的 探讨左卡尼汀对慢性肾衰竭血液透析患者心功能及血脂的影响.方法 将42例维持性血液透析患者分为治疗组（21例）和对照组（21例）.治疗组于透析结束前静脉注射左卡尼汀1.0 g,每周3次,对照组注射等量的生理盐水.观察患者6个月后血脂、血红蛋白和临床症状的变化,用彩色多普勒测定患者每分钟心搏量、左心室射血分数（LVEF）、左心室体积指数（LVMI）、左心室后舒张末期内径（LVDd）和E/A比值.结果 治疗组血脂改善,血红蛋白上升,精神状态、体力明显改善,心肌收缩力增强,心功能指数明显改善（P＜0.05）.结论 左卡尼汀可改善透析患者的心功能和血脂异常.

Effect of L-carnitine on cardic function in hemodialysis patients

Objective To investigate the effect on cardiac function of L-earnitine in hemodialysis patients.Methods Fourty-two patients on maintaining hemodialysis were divided into two groups,treatment group(n=21)and control group(n=21).1.0 g L-carnitine was infused to the patients in treatment group at the end of dialysis three times a week for 6 months.While normal saline was given to the control group. Hb,Tch,TG,HDL,LDL were measured,Cardiac function indexes such as stroke volume,cardiac output,LVEF ,LVMI,LVDd and E/A ratio were determined before and after treatment.Results In the treatment group,physical and mental status were significantly improved and the cardiac function was improved too(P<0.01).Conclusion L-carnitine can decrease serum lipids,increase Hb and improve cardiac function in patients on maintaining hemodialysis.
