地佐辛对小儿全麻苏醒期躁动的影响

摘    要:目的:观察地佐辛预防小儿全麻苏醒期躁动的临床效果。方法:选择全麻下行下腹部手术的患儿60例,年龄2~9岁,随机分为地佐辛组和对照组。地佐辛组在手术结束前约30 min静注0.1 mg/kg地佐辛,对照组注入等量生理盐水。记录患儿拔管后0 min、10 min、20 min及30 min的躁动评分和CHEOPS镇静评分;同时记录手术、麻醉、清醒和拔管时间及术后6 h不良反应发生率。结果:地佐辛组无/轻度躁动发生率明显高于对照组(77%vs 20%)(P<0.05),而中、重度躁动评分发生率地佐辛组明显低于对照组(23%vs 80%)(P<0.05);地佐辛组各时点CHEOPS镇静评分均明显低于对照组(P<0.05)。结论:小儿手术结束前约30 min静注0.1 mg/kg地佐辛可获得苏醒期良好的镇静效果,减少躁动发生,且不增加拔管时间和术后6 h不良反应发生率。

Abstract:Objective:To investigate the preventive effect of dezocine on emergence agitation during the recovery of general an-esthesia in children. Methods:Sixty children aged 2 to 9 years old undergoing lower abdominal surgery were assigned randomly into two groups with 30 cases in each. Dezocine group was injected intravenously with dezocine 0. 1 mg/kg 30 min before the end of surgery, while control group was received the same volume of normal saline. The incidence of emergence agitation and CHEOPS scores were e-valuated at the time point of 0 min,10 min,20 min and 30 min after extubation. The time of surgery,anesthesia,recovery and extubation were recorded. Adverse effects in each group were also recorded. Results:The incidence of no or mild emergence agitation was 85% in the dezocine group,which was higher than that in the control group (77% vs20%,P<0. 05). While the incidence of moderate and se-vere emergence agitation in the dezocine group was obviously lower than that in the control group (23% vs80%,P<0. 05). The scores of CHEOPS at each time point in the dezocine group were significantly lower than those in the control group (P<0. 05). There were no significant differences in the time of surgery,anesthesia recovery and extubation and the occurrence of adverse effects between the two groups. Conclusion:Injection of dezocine 0. 1 mg/kg 30 min before the end of operation produces a favorable sedation effect without prolongation of extubation and adverse sides during the emergence from general anesthesia in children.
