后腹腔镜肾上腺肿瘤切除术

目的探讨后腹腔镜切除肾上腺肿瘤手术应用价值及腹膜后腔建立方法的改进。方法回顾总结我院2003年10月至2006年3月后腹腔镜手术治疗102例肾上腺肿瘤病人的临床资料。其中肾上腺嗜铬细胞瘤切除29例，原醛症肾上腺腺瘤切除或肾上腺切除术52例，皮质醇症肾上腺腺瘤切除术21例。结果102例全部成功，手术时间60分钟．130分钟，平均90分钟，建立腹腔后间隙时间为3．7分钟，平均为4分钟，术中平均失血量80ml，术后平均住院5天。结论后腹腔镜手术安全、对患者创伤小、术后肠道功能恢复快，伤口愈合早。新的后腹腔间隙建立方法有效、快捷、方便，值得在临床应用中推广。

Posterior Laparoseopie Adrenaleetomy

Objective To discuss application of posterior laparoscopic adrenalectomy and improvement of the method used to make a space in posterior abdomen. Methods to collect clinical data of 102 patients who received posterior laparoscopic adrenalectomy because of adrenal diseases from October 2003 to March 2006, including 29 cases pheochromocytomia, 52 cases primary aldosteronism and 21 cases cushing disease. Results we succeeded in doing all operation and the average operation time is 90 minutes. The time used to make space of posterior abdomen is 4 minutes and average loss of blood volume is 80ml. The average recovery time after operation is 5 days. Conclusion posterior laparoscopic adrenalectomy is safe and do the minimal harm to patient. The advantage of posterior laparoscopic adrenalectomy lies in that the function of intestinal tract and incision recovers quickly. The new method of making posterior abdomen is proved to be effective,convenient and shortcut, we should apply it to our work.
