妊娠高血压综合征患者及其胎儿肾动脉血流的彩色多普勒超声检查

摘    要:目的应用彩色多普勒超声检测妊娠高血压综合征(PIH)孕妇及其胎儿的肾动脉血流变化。方法应用彩色多普勒超声检测120例正常妊娠孕妇及其胎儿和49例PIH孕妇及其胎儿肾动脉收缩期血流峰值流速(Vmax)、舒张末期血流流速(Vmin)、阻力指数(RI)。结果轻、中、重度PIH孕妇的肾动脉RI、Vmax明显高于正常妊娠组(P〈0.01),中、重度PIH孕妇的胎儿肾动脉RI、Vmax明显高于正常妊娠组,且RI增高程度与病情轻重有关(P〈0.01)。结论彩色多普勒超声能动态追踪观察PIH孕妇及其胎儿的肾动脉血流变化,对预测PIH孕妇状况及其胎儿宫内状态具有一定的临床应用价值。

Abstract:Objective To investigate renal artery hemodynamics of pregnancy induced hypertension(PIH)patients and fetuses using color Doppler flow imging(CDFI).Methods The peak-systolic velocity(Vmax),end-diastolic velocity(Vmin),resistance index(RI)of the renal artery in 49 PIH patients and fetuses were examined on CDFI and compared with 120 normal pregnant women and fetuses.Results The RI and Vmax of the renal artery in PIH patients including mild,moderate and severe,were significantly higher than those in the normal control(P<0.01).The RI and Vmax of the fetal renal artery in moderate and severe PIH group were significantly higher than those in the normal control(P<0.01).The change of RI was correlated with the disease activity.RI in severe PIH group was significantly the highest than those in other groups.Conclusions CDFI can accurately monitor the renal artery hemodynamics of patients and fetuses in PIH.It is a useful tool for the observation of staging,evaluation of prognosis and monitoring treatment in PIH patients in order to reduce the mortality of PIH patients and fetuses and to improve the perinatal outcome.
