超声造影在肝脏局灶性病灶良恶性肿瘤鉴别诊断中的应用价值

摘    要:目的:探讨在肝脏局灶性良恶性肿瘤鉴别诊断中超声造影技术的应用价值。方法回顾性分析91例接受超声造影检查的肝脏肿瘤患者93个病灶的临床诊断资料,对肝脏不同病变的超声造影表现情况进行比较。结果在本次研究中,共有89个超声造影结果准确,诊断率为95.7%。在93个肝脏肿瘤病灶中,有78个恶性肿瘤病灶,有15个良性肿瘤病灶。超声造影回声增强程度、增强均匀度、消退快慢和增强快慢在肝脏良恶性肿瘤中存在统计学差异(P<0.05)。在本次研究中,有肝转移癌( metastatic liver carcinoma ,MLC)病灶26个,有肝细胞癌( hepato cellular carcinoma , HCC)病灶52个,HCC和MLC在超声造影增强程度和增强均匀度上差异明显(P<0.05),在增强快慢和消退快慢上无明显差异(P>0.05)。结论在肝脏良恶性肿瘤的鉴别中超声造影有很重要的应用价值。

Application Value of Ultrasonic Imaging in the Differential Diagnosis of Liver Focal Benign and Malignant Tumors
Abstract:Objective To study the value of contrast-enhanced ultreasonography in the differential diagnosis of benign and malignancy tumors of focal liver .Methods Retrospective analysis of clinical diagnosis materials of 93 focal lesions in 91 pa-tients of with liver cancer were conducted ,ultrasonic imaging of liver lesions in different situation were compared .Results After contrast-enhanced ultrasound diagnosis to all lesions ,89 lesions with contrast-enhanced ultrasound results satisfied ,diagnostic rate was 95.7%.In 93 liver cancer lesions,there were 78 malignant lesions and 15 benign lesions,there were significant differences in contrast-enhanced ultrasound echo enhancement degree ,enhance evenness ,subside speed and increase speed between benign and malignant tumors of the liver (P<0.05).In this study,hepatocellular carcinoma (HCC) and liver metastatic carcinoma (MLC) were respectively 52 lesions and 26 lesions,there had significant differences in contrast-enhanced ultrasound enhancement degree and enhance uniformity between HCC and MLC (P<0.05),there had no significant differences in subside speed and increase speed(P>0.05).Conclusion Contrast-enhanced ultrasound has important value in the identification of benign and malignant liver tumors.
