腹腔镜直肠癌根治术和开腹手术直肠癌根治术的疗效及对患者胃肠功能影响的比较

目的 对比分析腹腔镜和传统开腹手术对直肠癌根治术患者的疗效及对胃肠功能的影响.方法 12例患者分为2组,腹腔镜直肠癌根治术组(LAP组)和开腹直肠癌根治术组(OP组),每组60例.对比分析2组患者手术指标,血清胃动素和胃泌素含量及并发症等情况.结果 LAP组与OP组相比,手术时间无差异性,但术中出血量、淋巴结清扫个数、术后进食时间、术后吗啡用量、术后镇痛时间及术后住院时间有差异性(P<0.05);腹胀排气时间、肛门排气时间、血清中胃动素和胃泌素有差异性(P<0.05);并发症发生率也有差异性(P<0.05).结论 腹腔镜直肠癌根治术可以减小手术创伤,加快恢复胃肠道功能,可能与提高胃动素和胃泌素的分泌有关.

Comparison of the Curative Effects and the Gastrointestinal Function of Rectal Cancer Laparoscopic Radical Surgery of Laparoscopic and Open Operation for Rectal Cancer

Objective To the analysis study and compare the curative effects and the gastrointestinal function of laparo -scopic radical surgery and open operation for rectal cancerrectal cancer radical of laparoscopic and open operation .Methods 120 patients were divided into two groups,laparoscopic group(LAP) and open operation group(OP),each group had 60 patients. Compared the surgery index,serum motilin,and gastrin,and complications of the 2 groups were compared.Results Compared with the OP group,LAP group There had no difference in operation time between the 2 groups,but there had difference thein num-ber of the lymph node ,intraoperative blood loss ,postoperative time of food intake ,postoperative morphine consumption ,postopera-tive pain time and postoperative hospital stay had difference ( P<0.05);Abdominal distension exhaust time ,anal exhaust time, serum motilin and gastrin had difference between the 2 groups(P<0.05) between the 2 groups;complication rates between the 2 groupsin the two groups also had difference (P<0.05).Conclusion Laparoscopic rectal cancer radical surgery for rectal cancer can reduce surgical trauma and ,accelerate the recovery of gastrointestinal function ,and may be related to increase of the secretion of motilin and gastrin .
