摘 要: | 目的 探讨2型糖尿病合并甲状腺癌的发生率及预后的相关影响因素.方法 以534例住院2型糖尿病患者为研究对象,回顾分析其空腹血糖、餐后2 h血糖、胰岛素(FINS)以及甲状腺彩超检查的结果,对2型糖尿病患者的甲状腺癌发生率及预后相关因素进行分析.结果 2型糖尿病患者甲状腺癌的发生率为19.7%,女性的发生率为12.4%,高于男性6.4%,且随年龄的增加甲状腺癌的发病率增加.单因素分析结果表明,影响2型糖尿病合并甲状腺癌患者3年生存率的主要相关因素为性别、年龄、病理类型、淋巴结转移、临床分期、病程和胰岛素依赖.多因素回归分析结果显示,病理类型、临床分期、病程、胰岛素抵抗是影响2型糖尿病合并甲状腺癌患者预后的独立因素(P<0.05).结论 2型糖尿病患者的甲状腺癌的患病率较高,病理类型、临床分期、病程、胰岛素抵抗可能是2型糖尿病患者甲状腺癌预后的独立影响因素.临床实践中,应及时对患者进行甲状腺癌筛查,早发现早治疗,以减少甲状腺癌的发生.

The Incidence and Prognostic Factors of Type 2 Diabetes Mellitus Patients with Thyroid Cancer |

Abstract: | Objective To explore the incidence and prognostic factors of type 2 diabetes patients with thyroid cancer . Methods 534 cases of hospitalized patients with type 2 diabetes were selected .Fasting blood glucose ,blood glucose 2 hours after meal,FINS and thyroid ultrasonography were retrospectively analyzed ,the incidence and prognosis factors of type 2 diabetes with thyroid cancer were analyzed .Results The onset of type 2 diabetes with thyroid cancer was 19.7%,the occurrence of female was 12.4%,higher than that of male ,6.4%,and the incidence of thyroid cancer increased with age .Univariate analysis showed that influence factors of 3-year survival rate were gender ,age,pathological type,lymph node metastasis,clinical stage,duration and in-sulin-dependent .Multiple regression analysis showed that pathological type ,clinical stage ,duration and insulin resistance were in-dependent factors for type 2 diabetes and thyroid cancer prognosis (P<0.05).Conclusion The prevalence of diabetes is higher in patients with thyroid cancer ,histological type ,clinical stage ,duration,insulin resistance may be independent factors for progno-sis of type 2 diabetes patients with thyroid cancer .In clinical practice ,patients should receive thyroid cancer screening ,early de-tection and early treatment can reduce the incidence of thyroid cancer . |
