肝动脉化疗栓塞术联合内皮素抑制剂灌注对乙型肝病病毒相关肝细胞癌的疗效分析

目的：探讨肝动脉化疗栓塞术联合内皮素抑制剂灌注对乙型肝病病毒相关肝细胞癌的疗效分析。方法经组织学或临床诊断确诊为乙型肝病病毒相关肝细胞癌患者30例，随机分为捏皮塑抑制剂联合肝动脉化疗栓塞术（ TA-CE）的观察组和单纯TACE的对照组，各15例，两组患者均接受至少2个周期相应治疗，比较治疗前后两组患者的血液AFP水平、疗效、不良反应发生以及肝外转移情况。对两组患者术前及术后1个月CT灌注参数进行对比。结果治疗后两组患者在疗效、血液AFP水平、不良反应情况对比上无明显差异（ P＞0．05）。术后观察组患者肝动脉灌注值（ALP）、血流量（BP）、肝脏灌注指数（HPI）参数显著低于术前水平（P＜0．05），且显著优于对照组（P＜0．05）。结论肝动脉化疗栓塞术联合内皮素抑制剂治疗乙型肝病病毒相关肝细胞癌，在术后CT灌注参数变化以及术后1年肝外转移发生率上均明显优于单独使用肝动脉化疗栓塞术治疗方法。

Efficacy of Hepatic Arterial Chemoembolization Combined with Endothelin Inhibitor Perfusion in the Treatment of Hepatitis B Virus Associated Hepatocellular Carcinoma

Objective To investigate the therapeutic effect of hepatic arterial chemoembolization ( TACE) combined with endothelin inhibitor perfusion in the treatment of hepatitis B virus related hepatocellular carcinoma .Methods Upon histological or clinical diagnosis confirmed 30 cases of patients with hepatitis B virus related hepatocellular carcinoma were randomly divided into the observation group treated with knead skin plastic inhibitor combined with TACE and the control group treated with TACE alone,each with 15 cases,the 2 groups of patients underwent at least 2 cycles of treatment.Blood AFP level,curative effect and adverse reaction occurrence and liver metastasis before and after treatment of the 2 groups were compared .CT perfusion parame-ters were compared between the 2 groups before and 1 months after operation .Results There was no significant difference in effi-cacy,blood AFP level and adverse reaction between the 2 groups after treatment(P>0.05).Postoperative hepatic artery perfusion value(ALP),blood flow(BP),hepatic perfusion index(HPI) parameters of observation group were significantly lower than those before operation(P<0.05),and were significantly better than the control group (P<0.05).Conclusion TACE combined with endothelial calmodulin inhibitors in the treatment of hepatitis B virus related hepatocellular carcinoma ,in postoperative CT perfu-sion parameters and postoperative 1 year extrahepatic metastasis rate are significantly superior to TACE alone .
