白蛋白结合型紫杉醇联合铂类治疗晚期妇科肿瘤的临床分析

目的 观察白蛋白结合型紫杉醇联合铂类治疗晚期妇科肿瘤的近期疗效和不良反应.方法 选取88例晚期妇科肿瘤患者,随机分为4组,溶剂型紫杉醇化疗组(A组),白蛋白结合型紫杉醇化疗组(B组),白蛋白结合型紫杉醇联合卡铂化疗组(C组),白蛋白结合型紫杉醇联合奥沙利铂化疗组(D组),每组22例.3周为1个疗程,直至疾病进展或出现不可耐受的不良反应,2个疗程后对治疗有效的患者进行评价.结果 白蛋白结合紫杉醇联合奥沙利铂对于晚期妇科肿瘤的治疗最有效,总有效率为63.7%;与C组相比,A组的疗效较低,差异有统计学意义(P<0.05),与D组相比,A组的疗效较低,差异有统计学意义(P<0.05);与D组相比,B组的疗效较低,差异有统计学意义(P<0.05);与D组相比,C组的疗效较低,但是差异无统计学意义(P>0.05).4组化疗方案的不良反应发生率,在白细胞减少、血红蛋白减少、中性粒细胞减少方面差异均无统计学意义(P>0.05);在血小板减少、肝功能损害、肾功能损害、消化系统反应、周围神经感觉异常、过敏反应方面,各组之间差异有统计学意义(P<0.05).结论 白蛋白结合型紫杉醇联合铂类治疗晚期妇科肿瘤的近期疗效较好,不良反应较少.

Clinical Analysis of Albumin-bound Paclitaxel Combined with Platinum-based Drugs for Advanced Gynecological Tumors

Objective To observe the short-term efficacy and adverse reactions of albumin-bound paclitaxel combined with platinum-based drugs for advanced gynecological tumors .Methods 88 cases of advanced gynecological tumors were selected and randomly divided into 4 groups,solvent-based paclitaxel group (group A),albumin-bound paclitaxel group(group B),albu-min-bound paclitaxel combined with carboplatin group ( group C ) , albumin-bound paclitaxel combined with oxaliplatin group (group D),each with 22 cases.Every 3 week was 1 course until the disease made progress or intolerable adverse reactions ap-peared.After 2 courses,patients who showed curative effects were evaluated .Results Group D showed the best efficacy with the total effective rate of 63.7%;group A had lower effective rate compared to group C and group D ,there had statistical significance ( P<0.05);group B had lower effective rate compared to group D ,there had statistical significance ( P<0.05);group C had low-er effective rate compared to group D ,there had no statistical significance (P>0.05).The adverse reaction rates of the 4 groups, leucopenia,hemoglobin reduction and neutrophilopenia showed no statistical significance (P >0.05);thrombocytopenia,liver damage,renal impairment,digestive system reactions,peripheral nerve paresthesia and anaphylaxis showed statistical significance (P<0.05).Conclusion Albumin-bound paclitaxel combined with platinum-based drugs for advanced gynecological tumors has good short-term efficacy and fewer adverse reactions .
