血浆γ-谷氨酰转移酶与冠心病关系的研究

摘    要:目的:探讨血浆γ-谷氨酰转移酶(gamma-glutamyl transferase,GGT)与冠心病(CHD)的相关性。方法纳入2011年3月~2012年7月山东菏泽市立医院胸痛患者220例,依据冠状动脉造影(CAG)结果分为CHD组(n=140)和对照组(n=80)。冠心病组依据临床表现和病情由轻到重分为稳定型心绞痛(SAP)组(n=58)、不稳定心绞痛(UAP)组(n=45)和急性心肌梗死(AMI)组(n=37)。所有入选者记录身高、体重、血压及是否吸烟等冠心病危险因素,采用酶法检测GGT、血脂、血糖等指标。采用二元logistic回归分析评估GGT与冠心病及其危险因素的相关性。结果 CHD组患者血浆GGT水平明显高于对照组(15.14±7.36)U/L,P<0.01];随着病情的加重,GGT也随之增加SAP组:(38.43±6.10)U/L,UAP组:(54.68±4.12)U/L,AMI组:(61.32±9.61)U/L],各组间GGT水平均有统计学差异(P<0.05);Logistic回归分析显示,GGT是冠心病发生独立危险因素(OR=4.333,95%CI:2.596~7.234)。结论 GGT是与冠心病轻重程度有关,是其独立危险因素。

Relationship between plasma gamma-glutamyl transferase and coronary heart disease

Abstract:Objective To discuss the relationship between plasma γ-glutamyl transferase (GGT) and coronary heart disease (CHD).Methods The patients (n=220) with chest pain were chosen from Mar. 2011 to Jul. 2012, and then divided into CHD group (n=140) and control group (n=80) according to the outcomes of coronary angiography (CAG). CHD group was further divided, according to clinical symptoms and conditions, into stable angina pectoris (SAP) group (n=58), unstable angina pectoris (UAP) group (n=45) and acute myocardial infarction (AMI) group (n=37). The risk factors of CHD including height, weight, blood pressure and smoking history were recorded. The changes of GGT, blood fat and plasma glucose were detected by using enzyme-linked immunosorbent assay (ELISA), and the correlation between GGT level and CHD or its risk factors was reviewed by applying binary Logistic regression analysis.Results In CHD group, GGT level was significantly higher than that in control group (15.14±7.36) U/L,P〈0.01], and as the exacerbating of disease condition, GGT also increased SAP group: (38.43 ±6.10)U/L, UAP group: (54.68±4.12) U/L and AMI group: (61.32±9.61) U/L,P〈0.05]. Logistic regression analysis showed that GGT was an independent risk factor of CHD (OR=4.333, 95%CI: 2.596-7.234).Conclusion GGT is correlated to the degree of CHD and it is an independent risk factor.
